TCM Case Managers can help you:

• Find affordable housing.
• Connect you to mental health and medical health care.
• Obtain entitlements including:
  • Social Security benefits
  • Social Services / Food Stamps
• Help you connect to local food pantries.
• Help you regain personal documents:
  • State issued identification card
  • Birth certificate
  • Health insurance card
  • Veteran Form DD214
  • Social Security Card
• Assist you with job search.
• Any other resources needed to assist you with gaining safety and stability in the community.
• Lunch available three days a week at our Wellness and Recovery Center.
